Forms to bring to your treatment consultation

The treatment consultation is an appointment with the nurse who will talk you through your treatment plan, dispense medication if you need any and show you how to use it, the nurse will be able to answer any questions you have about your treatment plan.

At this point the nurse will also go through all the consent forms required for your treatment. To make this process as efficient as possible please download the following forms and bring completed copies to your treatment consultation appointment.

• In vitro fertilisation (IVF) & intracytoplasmic sperm injection (ICSI) including freezing of embryos consent form
• Women's consent to treatment and storage form (IVF & ICSI)
• Men's consent to treatment and storage form (IVF & ICSI)
